German law changed in 2023 to allow doctors to issue Krankschreibungen (sick certificates) via video or phone consultation without ever seeing you in person. Several apps have built this into a ten-minute process. Most GKV health insurance plans cover the consultation at zero cost.
When This Works
Online sick certificates cover most common illnesses: colds, flu, back pain, headaches, gastrointestinal problems, mild mental health days. The doctor cannot certify you if the illness requires physical examination (broken bones, injuries, suspected infections needing a swab). For anything beyond standard illness, you still need an in-person visit.
Apps and Services
- AU-Schein.de: fastest option, appointments available same day. GKV insured pay €0; privately insured pay €15. Available in German.
- Doktor.de: video consultation within 15 minutes, GKV covered, German and some English-speaking doctors available.
- Teleclinic: covered by TK, Barmer, DAK, and most other GKV. Book on the app, consult by video, receive the certificate digitally.
- Kry: Swedish telehealth app, operates in Germany, some English-speaking GPs available. GKV covered.
- Doctolib: mainly appointment booking for in-person doctors but some offer video for same-day slots.
The Process
- Download the app or open the website.
- Enter your insurance card number (Versichertennummer) — on the front of your Krankenkasse card.
- Describe your symptoms in the chat or video call. A doctor joins within minutes to a few hours.
- If eligible, they issue a digital AU (Arbeitsunfähigkeitsbescheinigung — the formal name for a sick certificate). Your employer receives it automatically from your Krankenkasse since 2023 (eAU system); you don't need to print or post anything.
The eAU System
Since January 2023, sick certificates are transmitted electronically to employers. You tell your employer you're sick (Meldepflicht — required by your first day absent), and your Krankenkasse sends the certificate to them directly. Keep a copy for yourself regardless.
How Many Days Can Be Certified Online
One to three days initially, extendable. Most apps certify up to five days per online consultation. After five days of continuous illness, most doctors require an in-person visit to rule out something more serious.
What About Prescriptions
Doctors can issue e-prescriptions (eRezept) during online consultations for most standard medications. The prescription goes to an app (like Gesund.de or your Krankenkasse app) or you choose a pharmacy to receive it digitally. Pick up your medication with your phone at any partnered pharmacy — no paper slip needed.
