Germany’s Healthcare System: What Actually Happens When You Get Sick

Germany has one of the world’s best healthcare systems by most objective metrics — but understanding how it actually works when you need it prevents unpleasant surprises. Here is a practical guide.

Kassenzahnarzt vs. Privatzahnarzt

The two-tier system applies throughout. For GKV (public health insurance) patients, you see Kassenärzte — doctors with GKV contracts. They provide all standard treatments covered by public insurance. Private patients (PKV) or GKV patients willing to pay out-of-pocket can see the same doctors but access private billing rates — potentially shorter waiting times, private rooms if hospitalised, and treatment directly from the chief physician (Chefarzt) rather than a resident.

The GP as Gatekeeper

Your Hausarzt (GP/family doctor) is the first contact for non-emergency issues. For specialist referrals (to a cardiologist, dermatologist, neurologist, etc.) you generally need a GP referral — though specialists can be seen directly if you book ahead. The Hausarzt maintains your overall health record and coordinates care. Finding and registering with a Hausarzt shortly after arriving in Germany is important — popular practices close their patient lists.

Emergency vs. Urgent Care

Emergency (Notaufnahme): go to the nearest hospital for life-threatening situations. Call 112 for ambulance. For urgent but non-life-threatening issues outside office hours, the Kassenärztliche Vereinigung operates an emergency phone service (116 117) that can direct you to the nearest on-call doctor or overnight clinic. Many pharmacies also maintain an on-call rotation (Notdienst) accessible by search or via the pharmacy’s own posted notice.

Prescription System

Prescriptions (Rezept) from GKV doctors come in two forms: standard green Rezept (GKV-covered, you pay a €10 co-payment per item up to the medicine cost) and private Rezept (full cost plus pharmacy markup). For GKV patients, the standard co-payment applies to most standard medications; some categories are exempt (children, those with low income). The KV system means standard medication costs are highly regulated and generally affordable.
