mRNA Technology Platforms: Evolution from COVID Vaccines to Cancer Treatment and Future Applications

mRNA (messenger RNA) vaccine technology made a historic leap from laboratory to billions of vaccinations during the COVID-19 pandemic, profoundly changing the vaccine development paradigm. Core mRNA advantage: no protein or attenuated virus production required — just design and synthesize the mRNA sequence; after injection, cells translate it into the antigen protein themselves. Speed is dramatic: COVID mRNA vaccines went from sequence design to clinical trials in under one year.

## Mechanism

mRNA vaccines encapsulate mRNA encoding the target antigen protein in lipid nanoparticles (LNPs). LNPs protect mRNA from nuclease degradation and help mRNA cross cell membranes into the cytoplasm, where ribosomes translate it into the antigen protein, triggering immune system antibody and T cell responses. Key technology: modified nucleotides (N1-methylpseudouridine, N1mΨ) significantly reduced mRNA immunogenicity (reducing non-specific inflammation) — the core contribution of Katalin Karikó and Drew Weissman, for which they received the 2023 Nobel Prize in Physiology or Medicine.

## Pipeline Expansion: Beyond COVID

**Influenza vaccines**: Moderna’s mRNA flu vaccine (mRNA-1010) showed superior immunogenicity over traditional flu vaccines in Phase III — potential 2025–2026 approval. Rapidly sequence-updatable mRNA flu vaccines can fundamentally solve traditional flu vaccines’ “prediction” problem of guessing the seasonal strain.

**RSV vaccine**: Moderna mRNA-1345 received FDA approval (2024), the second approved mRNA vaccine product after COVID.

**Personalized cancer vaccines (mRNA-4157/V940)**: BioNTech/Moderna co-development customizing mRNA sequences per patient’s tumor mutation profile (encoding 20 tumor neoantigens) to activate T cells to attack the tumor. Phase II data showed significant efficacy in melanoma combined with PD-1 inhibitors.
