Statutory health insurance (gesetzliche Krankenversicherung, GKV) is mandatory for most employees and students in Germany. While basic coverage is identical across all GKV providers (mandated by law), the additional benefits, service quality, and digital experience vary significantly.
The Universal Baseline
All GKV insurers must provide: general practitioner visits, specialist referrals, hospital treatment, prescription medications (with co-pay), dental checkups (annual), preventive screenings, sick leave certification (Krankmeldung), and emergency care. These are identical regardless of which Krankenkasse you choose.
Where They Differ: Zusatzleistungen (Additional Benefits)
Techniker Krankenkasse (TK): consistently ranked #1 in customer satisfaction. Strongest digital services: full English-language app, online doctor video consultations, digital prescriptions, broad international coverage while traveling. Additional benefits: comprehensive preventive programs, mental health support, extensive bonus program (up to €270/year for preventive health activities). Best for: international students who need English support and digital convenience.
BARMER: strong second. Excellent digital app, good English support, strong mental health programs. Similar bonus program to TK. Slightly better in some regional areas than TK.
AOK: regional insurers (AOK Bayern, AOK Nordost, etc.) — coverage and quality varies significantly by region. Often better for local health programs and regional specialists. Weaker English-language support than TK/BARMER.
DAK Gesundheit: national provider with competitive rates, good preventive programs. Slightly weaker digital experience than TK. Strong in dental additional coverage.
Student Health Insurance
Students under 25 (or 30 in some cases) pay a reduced rate: ~€123/month (2025). All GKV providers offer student rates. You must enroll within 3 months of beginning your studies — missing this window can result in gaps or forced private insurance. International students: you must provide proof of German GKV or recognized equivalent insurance for enrollment at German universities.
Switching Krankenkasse
You can switch Krankenkasse with 2 months notice (since 2021, the waiting period was reduced from 18 months to 2 months). Switch timing: after 12 months with your current provider. To switch: sign up with the new provider, they handle the cancellation of the old one automatically.
