Start Here: The Psychotherapeutische Sprechstunde
Before you join any waiting list, book a Psychotherapeutische Sprechstunde (initial consultation). You need no referral and no Hausarzt. These slots assess your situation and count separately from your therapy quota. Every licensed therapist must offer them. Call directly, say "Ich möchte eine Sprechstunde buchen" (I'd like to book an initial consultation), and you usually get an appointment within two to four weeks.
Finding an Insured Therapist
- psychotherapiesuche.de — the most complete national database. Filter by Kassenpatient (insured patient), your city, and specialty.
- jameda.de — shows ratings and wait time estimates. Filter "Kassenpatient" before searching.
- Your Krankenkasse hotline — TK (Techniker Krankenkasse) has an English mental health support line. Barmer and AOK can send you a local list within 24 hours.
- Terminservicestelle (TSS) — call 116117. By law, the TSS must find you an appointment within four weeks. If they fail, you can see a private therapist and bill your insurance.
Free Options That Work Faster
The Psychosoziale Beratungsstelle (PSB) at your university offers free short-term counseling, typically within two to three weeks. You must be enrolled. Most German universities have one — search "[your university] PSB" or "Psychologische Beratung Studium."
The national crisis line (Telefonseelsorge) runs 24 hours, costs nothing, and has German-speaking staff: 0800 111 0 111 or 0800 111 0 222.
For Chinese-speaking therapists, search "chinesischsprachige Psychotherapeuten" on jameda.de. Berlin, Munich, Frankfurt, and Hamburg all have practitioners who speak Mandarin.
If You Can't Wait Six Months
Private therapists (Privatpraxis) take new patients within one to two weeks. Sessions cost €80 to €150. You pay upfront, then apply to your GKV for reimbursement under the Kostenerstattungsverfahren. To qualify, collect written rejection letters from at least three insured therapists, proving no slot was available within a reasonable time. TK, Barmer, and most other Kassen approve these claims when documented properly.
Online therapy apps like TheraPly and Selfapy provide structured programs, and some GKV plans cover part of the cost. Check your Krankenkasse's app or website for "Digitale Gesundheitsanwendungen" (DiGA) — approved digital therapeutics they fund at zero cost to you.
What to Tell Your Hausarzt
Your GP can issue a Krankschreibung (sick certificate) for mental health reasons without requiring a diagnosis. Say: "Ich habe psychische Beschwerden und brauche Unterstützung." They can also write you a Überweisung (referral) to a psychiatrist if medication might help. Psychiatry wait times are shorter than psychotherapy — three to eight weeks on average.
